Learn More About Our Services

Liberty POST’s Early Intervention Ongoing Service Coordinators (OSCs) help parents and families identify areas of concern, form a plan to address them with a variety of services and therapies, and then ensure plans are followed.

OSCs use their community connections to help families access the programs and services their children need. They also use their expertise to advocate for children with developmental delays and differing abilities in school or other environments outside the home.

Families can also rely on OSCs to assist them with obtaining anything related to their child’s intervention plan, including health information, health insurance, evaluations, or appointments.

Liberty POST offers physical therapy tailored to the individual needs of each child, helping them to improve their gross motor skills, which include large, sweeping motions with their arms, legs and torso. This allows for greater independence in moving around their world.

Physical therapy can include exercises to improve strength, endurance, muscle tone, balance, and coordination, adaptations of daily care and routines, and equipment fitting and assessment.

A child’s physical therapy can occur anywhere it would be most useful to them. This may be at home or in their community. Our physical therapists always strive to make activities and exercises feel less like work and more like play.

Occupational therapy at Liberty POST uses proven strategies and play-based methods to help children enhance their small muscle movement and daily living/functional skills.

We want to ensure that the intervention plan we develop in occupational therapy is practical for both the child and their family, which is why we always encourage the input and participation of caregivers. Our occupational therapists will work with you and your child to identify and address any areas of concern associated with fine motor skills, the sensory system, self-care skills, and visual motor integration.

This therapy focuses on communication (understanding and use of language and sound production), voice, feeding and swallowing, auditory processing, and social language.

Speech and language therapy helps individuals reach their goals with direct therapy and practical techniques, including breath support, feeding and swallowing, and forming words – just to name a few. Additional therapies may include the use of visual, verbal, and/or tactile cues.

Our board-certified musical therapists support social, cognitive, motor skills, and behavioral development through a variety of musical activities. Children are given the tools and encouragement to express themselves while working coordination, cognitive processing, interpersonal skills, attention span, and memory.

A Children’s Health Home is a care management service model where ALL of a child’s providers communicate with one another so that a family’s needs are addressed in a comprehensive manner.

Liberty POST is a member of the Children’s Health Home of Upstate New York (CHHUNY) and has aided children and families through this journey for nearly 15 years, making us leaders in the evaluation, diagnosis, special education and therapeutic services.

As the child enters preschool, they can now remain with the same care coordinator providing continuity of services for both the child and their families.

Our Health Homes Care Managers work with families to navigate the complicated, and often bewildering, regulatory system of school districts, health care, and Medicaid.

Liberty POST provides Diagnostic Evaluations to determine a possible diagnosis of Autism Spectrum Disorder (ASD) or other formal diagnoses such as Attention Deficit Hyperactivity Disorder (ADHD). After diagnosis, the team – in collaboration with the child’s family and support network – helps to develop individualized plans that explore a variety of therapies, interventions, and techniques.

Interventions are tailored to the needs, learning styles, play preferences, and areas of concern of each individual.

Our program is designed to build confidence and enhance a child’s innate ability. We take a holistic approach that involves in-person therapy, coaching, and a plan for at-home sessions, which parents can choose to record and send to our clinicians for feedback.

Our Liberty POST psychologists assess at your child’s development through a global perspective and are able to provide our clinicians with guidance.

We will work with you and your child to identify your concerns and goals, as well as the different services and strategies that can be used to effectively address them. Our psychologists can evaluate for a delay or diagnosis, coordinate with physicians and schools, and determine which services your child may need.

At Liberty POST, the role of our social workers is to partner with each member of a child’s support system – including their family, school, and service providers – to navigate barriers to their development, growth, and overall well being.

Our social work team focuses on building therapeutic relationships while advocating for each child’s needs, including any additional support or clinical services. They will also develop a plan for intervention and provide individual or family therapy, if needed.

Our social workers can assist children and families with a wide range of hands-on assistance and advocacy.

A registered dietitian will assess your child’s nutritional needs, set goals and desired outcomes, and then create a sustainable plan. This will ensure they can reach those goals and continue to meet their nutritional needs.

Whether it’s a referral to medical care, diet supplementation, or developmental monitoring, we have the expertise and direct experience necessary to assist families and children with interventions and strategies.

Area Programs

Specialized Regional Programs

In addition to our services, we offer specialized programs in select regions across the state.

Children in the Syracuse region receiving Early Intervention services through Liberty POST may be eligible for Assistive Technology (AT) services, which can include physical modifications of the child’s home environment or equipment, devices, or utensils that allow them to increase participation in daily routines and activities.

Children in the Buffalo, Syracuse, Hudson Valley, and Long Island regions receiving services through Liberty POST have access to Sensory Gyms. Pediatric therapists work with children either in a group or individually to build and enhance foundational skills through play. Suspended swings, rolling scooters, climbing walls, and ball pits are all used as tools to support development.

Children receiving Liberty POST services in the Northern New Jersey region may participate in structured social groups. Social groups help children strengthen their social skills and learn how to make friends by practicing taking turns, show and tell, and everyday skills like listening, sharing, and apologizing.

STARS Preschool is a Liberty POST program designed for children ages 3 to 5 of all abilities, with or without special accommodations. The curriculum follows New York state educational requirements through play, movement, social interactions, and music. STARS Preschool is available in the Rochester and Syracuse regions.

Preschool Solutions is a private, community preschool available to children ages 2 to 4 years old in the Hudson Valley region. This program encourages a love of learning with enriching activities such as circle time, small group learning, thematic activities, movement, and social interactions. We strive to connect with our students and give them fundamental tools for success.
